As our receptionist, you perform clerical support in an efficient manner in accordance with established procedures, and as directed by your supervisor. Experience & Education: 
  • A minimum of two (2) years experience in a clerical position, or successfully completed a secretarial science program from an accredited school/college, is preferred.
  • Must have a working knowledge of filing and respect for confidentiality.
Duties & Responsibilities:
  • Operate telephone switchboard.
  • Utilize proper phone etiquette.
  • Receive and direct all persons entering the facility to appropriate areas.
  • Responsible for general typing and secretarial functions.
  • Distribute paychecks per policy and procedure.
  • Is responsible to receive and direct incoming mail and deliveries appropriately.
  • Demonstrate correct safety techniques.
  • Respect and take care of facility equipment and not wasteful of supplies.
  • Contribute to and promote resident and family relations.
  • Follow facility’s policies and procedures.
  • Continue growth and expand job knowledge.
  • Demonstrate leadership skills.
  • Other duties as assigned by manager.
Specific Requirements:
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action and to remain calm during emergency situations.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public.
  • Must be knowledgeable of administrative practices, procedures and guidelines.

As the MDS Coordinator, you plan, organize, and direct the MDS process in accordance with federal and state standards, guidelines, and regulation that govern our facility. Experience & Education:
  • Must possess a current, unencumbered license to practice as an RN in this state.
  • Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university, RN preferred
  • Must be able to coordinate MDS systems, resident assessment and care plans for each resident timely.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
Duties & Responsibilities:
  • Coordination of MDS process.
  • Oversees ADL training for facility and staff.
  • Identifies data integrity weaknesses in medical records and communicates findings.
  • Complete and transmit all CMS approved item sets (MDS)
  • Attendance at RCCs to lead discussion (OBRA only)
  • Attend morning report, maintain communication and rehab services, nursing staff and others to keep abreast of resident condition change.
  • Complies with facility privacy policies and procedures and protects residents’ individual health information.
  • Issues and delivers Medicare BN (denial letters) per CMS regulation.
  • Maintains adequate systems to ensure appropriate document (i.e., MD certs, IDT program, insurance authorization)
  • Oversees scheduling of MDA and Care Conference.
  • CP review- Annual, sig chg, Qtrly*
  • *Care Plan review includes reviewing existing care plans provided by unit nurses on admission through the assessment period. Adding care plans for those additional issues triggered through the RAI process. Episodic care plans are added by the unit nurse at the time of the event. Episodes include an event that would alter the current care plan of the resident.
Specific Requirements:
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.

  • Must be knowledgeable in the privacy rule, comply with facility’s privacy practices, train staff in the same and provide additional training if changes are made to policies and/or there is staff non-compliance.
  • Must possess leadership and supervisory ability and willingness to work harmoniously with professional and non-professional personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures, etc., that are necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to communicate effectively to appropriate personnel regarding emergency situations.
  • Must possess accurate and comprehensive assessment skills to ensure standards of nursing practice.

  • Educate department staff in all aspects of dietary functions.
  • Assure the planning, preparation and serving of all therapeutic diets.
  • Oversee that sufficient inventory is maintained according to policy and procedure including emergency supply.
  • Formulate menu planning within accepted therapeutic guidelines.
  • Assure that menus are accurately adhered to and appropriate substitutes are made per policy.
  • Ensure quality and quantity of all food delivery.
  • Demonstrate correct safety techniques.
  • Is knowledgeable in and ensures staff comply with privacy practices to protect residents’ health information.

We are hiring a Hospice Social Worker to join Hospice of Western & Central Massachusetts in Worcester! This is an on-the-road position. Shift is 8a-4:30p.   • Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.  Provides an assessment in the patient’s identified residence and assistance when this is not safe and another plan is required.
  • Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plans intervention based on evaluation findings.  Counsels’ patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties.  Provides crisis intervention when necessary.
  • Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family.  Includes communication, role of family, space, and any special traditions or taboos.
  • Maintains clinical records on all patients referred to social work.
  • Educates patients and families on, and assists in, preparation of advanced directives.
  • Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
  • Provides information to patients and families/caregivers and community agencies.
  • Serves as liaison between patients and families/caregivers and community agencies.
  • Maintains collaborative relationships with organization personnel to support patient care.
  • Maintains and develops contracts with public and private agencies as resources for patients and personnel.
  • Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
  • Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.
Experience & Education: 
  • A graduate of a master’s program in social work (MSW) accredited by the Council on Social Work Education.
  • Experience in health care, hospice experience preferred.
  • Understands hospice philosophy, and issues of death/dying.
  • Strong background in mental health/counseling and experience in health services delivery.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Duties & Responsibilities:

  • Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
  • Reviews patient's medical diagnosis, prognosis, medications, procedures and clinical course.
  • Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
  • Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
  • Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel.
  • Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals.  Assists in the orientation of new employees.
  • Assists Clinical Director and other supervisory hospice personnel in the planning, implementation, and evaluation of Inservice and continuing education programs.  Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals, and objectives, both short and long range.
  • Complies with accepted professional standards and principles.
  • Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs.  Participates in public relation and community activities that promote the organization's role as an effective member of the hospice and health care delivery system.  Promotes customer service orientation to all hospice personnel.
  • Participates actively in quality assessment performance improvement teams and activities.
  • Performs other duties and activities as delegated by the Clinical Director.

  • Ability to travel.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
