We are hiring a Memory Care Program Coordinator to join our dynamic care team at Cape Regency Rehab in Centerville! Weekend or evening coverage may be needed at times. As the Memory Care Program Coordinator, you enhance the overall operation of the activity department by developing/implementing the Dementia specific programming to ensure that an ongoing program of activities designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental and psychosocial well-being of each resident is maintained. Education and Experience:
  • One (1) year experience in a supervisory capacity in a hospital, skilled nursing care facility or other related medical facility activities department is preferred.
  • Experience working with those diagnosed with Dementia or Alzheimer’s is required.
  • A facility operating one or more DSCUs shall have a therapeutic activity director for the DSCU(s) who is a qualified therapeutic recreation specialist or an activities professional who:(1)   Is certified as a therapeutic recreation specialist or as an activities professional by an accrediting body recognized by the Department; (2)   Has two years of experience in a social or recreational program within the last five years, one of which was full-time in a resident activities program in a health care setting; or (3)   Is a qualified occupational therapist or occupational therapy assistant.
Duties and Responsibilities:
  • Assist in plans and provisions of patient care by visiting residents regularly to evaluate needs.
  • Document and report to the nursing department any unusual or significant findings.
  • Encourage resident to take part in activities of daily living that will stimulate his/her potential, physically, mentally, socially and spiritually.
  • Spend duty time with residents rather than with other facility staff.
  • Assist with and follow through on the establishment of resident care goals within the framework of the entire therapeutic team.
Specific Requirements:
  • Must possess sensitivity to those diagnosed with Dementia and their families.
  • Must have an understanding of appropriate activities for those with dementia.
  • Must be able to read, speak and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
  • Must be knowledgeable in the Privacy rule and ensure all staff comply with facility privacy practices policies and procedures.
  • Must possess leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing nursing principles.
  • Must be able to relate information concerning a resident’s condition.
  • Must participate in continuing education and inservice training.

  • Evaluates the status of Infection related practices in the facility via monthly surveillance rounds.

  • Coordinates and reports on the Annual OSHA Sharps survey.
  • Maintains Employee Health Records

  • Reviews Monthly Laboratory reports, assures up to date laboratory policy/procedure manuals are available. Reviews the annual antibiogram.

  • Coordinates employee annual TB symptom surveys; if annual employee Tuberculin skin tests are required, will assure their completion.
