As the Director of Social Services, your role is to plan, organize, develop, and direct all required social services to meet the needs of residents in accordance with federal, state, and local standards, guidelines, and regulations that govern our facility and as may be directed by the Administrator to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. Experience & Education:
  • A minimum of one (1) year social work in a hospital, skilled facility, or other health-related field preferred.
  • Must possess, as a minimum, a bachelor’s in social work from an approved school of social work.
  • For a Director of Social Services role, a master’s in social work is preferred.
Duties & Responsibilities:
  • Coordinate and monitor resident care conferences.
  • Work with individual residents, responsible parties, and families towards adjustment to placement and to crisis (i.e., illness, disability, and death).
  • Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information.
  • Complete comprehensive documentation in a timely manner.
  • In the role of advocate coordinate personal needs.
  • Utilize community resources appropriately.
  • Support staff in relation to resident-related issues.
  • Demonstrate correct safety techniques.
  • Respect and take care of facility equipment and not wasteful of supplies.
  • Contribute to and promote positive resident and family relations.
  • Follow facility policies and procedures.
  • Other duties deemed appropriate and assigned by your supervisor.
Specific Requirements:
  • Must be able to read, write, speak and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
  • Must have patience, tact, cheerful disposition, and enthusiasm, as well as being willing to handle residents based on whatever maturity level in which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing Social Service practices.
  • Must be able to relate information concerning a resident’s condition/incident to the Interdisciplinary Team as appropriate.

  • Be able to follow written and oral instructions.

  • Is knowledgeable in various payment sources as needed and assists resident and families.
  • Is diligent in collections.

As an Activities Assistant, you assist the Director of Therapeutic Recreation and/or Memory Care Program Coordinator in planning, organizing, developing, and directing the overall operation of the Therapeutic Recreation Department to ensure that an ongoing program of activities designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental and psychosocial well-being of each resident is maintained. Experience & Education: 
  • Must possess, as a minimum, a high school diploma or its equivalent.
  • As a minimum, one (1) year experience in an Activities Department in a hospital, skilled nursing care facility or other related medical facility is preferred.
Duties & Responsibilities:
  • Assists in plans and provisions of patient care by visiting residents regularly to evaluate needs.
  • Document and report to the nursing department any unusual or significant findings.
  • Report all reportable events immediately in accordance with our policy.
  • Encourage residents to take part in activities of daily living that will stimulate his/her potential, physically, mentally, socially, and spiritually.
  • Spend duty time with residents rather than with other facility staff.
  • Assist with and follow through on the establishment of resident care goals within the framework of the entire therapeutic team.
Specific Requirements:
  • Must be able to read, write, speak and understand the English language.
  • Must be at least 18 years old.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must be knowledgeable in the Privacy Rule and ensure all staff comply with facility privacy practices policies and procedures.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level at which they are currently functioning.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing policies and procedures.
  • Must be able to relate information concerning a resident’s condition.

  • Develops and implements a nursing service organization structure.
  • Determines the staffing needs of the nursing service department, making recommendations to the Administrator, necessary to meet the needs of the residents.
  • Oversees licensed nursing staff
  • Develops work assignments and schedules duty hours, and/or assists nursing supervisor staff in completing and performing tasks necessary to ensure quality of care.
  • Develops and maintains an on-call program for the nursing department.
  • Makes daily rounds of each nursing unit and does visual inspection on each unit.
  • Reviews satisfaction of care and services with interviews with families and residents.
  • Assists the Infection Preventionist in establishing and maintaining the Infection Control Program.
  • Assists the Director of Staff Development in ensuring that all mandatory in-service education sessions, which are required by state and federal law, are performed per schedule; documented and such documentation is retained.
  • Assists the Administrator in developing safety standards for the nursing department and monitors these through employee incidents and develops a plan of correction and audit tool for prevention. The Director of Nurses is an active member of the Safety Committee.
  • Abides by all reporting policies and measures set forth by Athena Health Care Systems.
