As the Human Resources Director you would be responsible for planning, coordinating, managing, and directing all activities and programs relating to the recruitment, selection, retention, orientation and development of employees at the four Athena Home Health and Hospice agencies in Connecticut, Massachusetts and Rhode Island.  You would act as an advocate for organization personnel and collaborates with Home Health and Hospice executive leadership as well as Athena Healthcare Systems Human Resource Management team. Experience & Education:
  • Bachelor’s degree in business administration or related field, master’s degree preferred.
  • At least three (3) years’ experience in health care management, preferably in hospice care operations. Two years of experience in employee recruitment and training preferred, and employee and labor relations.
  • Has knowledge of state, federal and CHAP regulations relating to employment and employee education.
  • Has knowledge of corporate business management.
  • Demonstrates good written and oral communication skills and public relations skills.
  • Demonstrates autonomy, organization, assertiveness, and flexibility and cooperation in performing job responsibilities.
Duties & Responsibilities:
  • Directs all daily human resources operations including providing direct oversight of the establishment and implementation of human resource policies.
  • Ensures compliance with all state, federal, and CHAP human resources regulatory requirements.
  • Directs the implementation of improved work methods and procedures to ensure that consistent employee recruitment, selection, retention, hiring practices, orientation and termination procedures are handled in accordance with applicable law and regulation.
  • Monitors competitive market and makes recommendations to propose to the Executive Director for compensation adjustments to ensure that the Organization's Name is competitive in the market.
  • Works with the management team to identify recruitment needs and develops a recruitment plan designed to fill demand and evaluates the cost-effect of recruitment efforts.
  • Builds and monitors community perceptions of Organization's Name as a high-quality employer.
  • Assists the Executive Director/Administrator in the preparation of the annual budget for the human resources department and monitors the allocation of resources according to budgetary limitations.
  • Coordinates and participates in community awareness and recruitment activities such as health fairs.
  • Works with the management team in identifying employee education needs and develops orientation and continuing education programs for all employees.
  • Recruits, selects, and directly manages members of the human resource team.
  • Maintains a comprehensive library of resources to support employee education and development.
  • Maintains confidentiality of all employee information and files.
  • Annually monitors personnel satisfaction with benefits and work environment and reports the results per organization practice.
  • Makes recommendations regarding organization, personnel benefits package, and cost-effective plans.

In this role, you plan, organize, and direct hospice care and are experienced in nursing, with an emphasis on community health education/experience.  The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Experience & Education: 
  • Graduate of an accredited school of nursing.
  • Current licensure in State and CPR certification.
  • Bachelor’s degree, with one (1) year of hospice care experience preferred.
  • Ability to travel. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Duties & Responsibilities:
  • Respond to on-call messages that will include making recommendations via phone and making visits to patients about whom the call is received.
  • Make scheduled visits and/or admission visits to patients as directed by the Clinical Supervisor; includes visits to patients admitted the previous day, and any patients with a clinical need for a visit.
  • Provide appropriate support at time of death and perform bereavement assessment if requested by the Clinical Supervisor.
  • Perform death pronouncements for patients that have passed away in any location where there is not a registered nurse available to pronounce the death.
  • Completes a comprehensive assessment of the patient and family to determine hospice eligibility and care needs.  Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient’s status by:
    1. Writing and initiating plan of care
    2. Re-evaluating patient and family/caregiver needs
    3. Participating in revising the plan of care as necessary
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
