We are hiring Hospice Certified Nursing Assistants (CNAs) to join our team in Rhode Island! Athena Health Care Systems has been a leader in providing quality health care services since 1984. PURPOSE OF POSITION: Carries out duties assigned by RN Case Manager to increase the level of patient comfort, to maintain good personal hygiene and a safe environment for the patient. ESSENTIAL FUNCTIONS:
  • Adheres to the written home health aide plan of care as prepared by the RN Case Manager.
  • Promptly reports changes in the patient’s condition to the RN Case Manager.
  • Assists the patient with all aspects of personal care, ambulation and positioning as required.
  • Prepares meals and assists with feeding as required.
  • Performs household services which are necessary for the health and safety of the patient and family.
  • Provides assistance with bedpans and urinals as needed.
  • Provides guidance to the caregiver regarding personal care of the patient.
  • Attends and participates in weekly team meetings, AM report, in-services, and agency meetings and other related activities when appropriate.
  • Understands and adheres to Athena Hospice of Rhode Island policies and procedures.
  • Follows the guidelines for universal precautions when providing patient care.
  • Maintains records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by Athena Hospice of Rhode Island.
  • Submits legible and accurate documentation of all services provided in a timely manner in accordance with home health aide documentation requirements.
  • Carries out duties and responsibilities of position in a manner that exemplifies excellent customer service.
  • Consistently represents Athena Hospice of Rhode Island to all internal and external customers with integrity and professionalism.
  • Encourages and maintains safety standards in the workplace and home.
SPECIFIC REQUIREMENTS:
  • Must pass the Home Health Aide competency exam.
  • Must be proficient in English to the extent of being able to read and understand instructions and communicate effectively with clients.
  • Must understand and use universal precautions when dealing with clients.
  • Must have access to an automobile and must have a valid driver's license.
  • Must be able to physically perform personal care on clients.
  • Must have a valid CNA license.
WORKING ENVIRONMENT: Works out of the Athena Hospice of Rhode Island office with extensive travel required to patient’s homes and/or nursing facilities. PURPOSE OF POSITION: Responsible as a member of the Interdisciplinary Team for the provision of nursing care for hospice patients to maximize the comfort and health of patients and families consistent with Athena Hospice of Rhode Island’s policies and procedures. ESSENTIAL FUNCTIONS: · Maintain communication with other hospice staff members and other agencies as needed to provide optimal care and use of resources for the patient/family. · Maintain regular communication with the RN Case Manager or Team Leader to review caseload. · Implement the plan of care as specified by the Interdisciplinary Team. · Provide personal care as needed. · Perform nursing procedures as ordered. · Determine the patient's needs and report findings to the RN Case Manager. · Recommend changes to the plan of care as appropriate. · Complete accurate charting of findings, treatments, and patient responses. · Organize care to provide timely and efficient care to all patients. · Supervise nursing aids. · Provide emotional support to patients and their family and friends. · Follow strict infection control guidelines. · .Participate in agency and community programs as requested to promote professional growth and understanding of hospice care. · Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which assist in determining policies. EDUCATIONAL REQUIREMENTS Possession of current license to practice as a registered professional nurse in the state of Rhode Island. Preferably a B.S. degree in nursing. Minimum two years of varied work experience as a professional nurse and preferably one of the two years in an acute or hospice setting. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. EXPERIENCE A minimum, one (1) year experience in a hospital, long-term care facility or other related health care facility is preferable.
