The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Experience & Education: 
  • Graduate of an accredited school of nursing.
  • Current licensure in State and CPR certification.
  • Bachelor’s degree, with one (1) year of hospice care experience preferred.
Duties & Responsibilities:
  • Responsible for supervising hospice aides.
  • Completes an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs.  Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of the nursing process.
  • Assesses and evaluates patient’s status by:
    1. Writing and initiating plan of care
    2. Regularly re-evaluating patient and family/caregiver needs
    3. Participating in revising the plan of care as necessary
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

Duties & Responsibilities:
  • Respond to on-call messages that will include making recommendations via phone and making visits to patients about whom the call is received.
  • Make scheduled visits and/or admission visits to patients as directed by the Clinical Supervisor; includes visits to patients admitted the previous day, and any patients with a clinical need for a visit.
  • Provide appropriate support at time of death and perform bereavement assessment if requested by the Clinical Supervisor.
  • Perform death pronouncements for patients that have passed away in any location where there is not a registered nurse available to pronounce the death.

Duties and Responsibilities:
  • Act as a liaison between the referral accounts/the community and Athena Home Health and Hospice to identify and develop new potential referral sources.
  • Develop and maintain a monthly and daily marketing plan based on the Home Health and Hospice needs.  Service accounts in accordance with the marketing plan and as needed.  Responsible for timely and accurate completion of monthly and daily marketing/business development action plans.
  • Maximizes census through effective marketing, sales calls, and public relations in accordance with the Home Health and Hospice goals.
  • Effectively develop and implement monthly marketing plans that are organized and strategic through geographic planning, completion of daily marketing/admission action plans, targeting account assignments, adherence to account assignments, sales call frequency, strategy for account development, and vertical account development.
  • Ability to demonstrate technical skills which support/enhance sales skills and results through: sales delivery, collateral use, product knowledge, knowledge of competitors, identifying challenges and creating/initiating solutions in a positive and effective way.
  • Responsible for external sales calls/contact with assigned referral accounts based on account priority listing.
  • Responsible to ensure external sales calls effectively and successfully demonstrate preparation, discovery, presentation of features/benefits, handling of objections, and closing skills.
  • Create and distribute collateral materials (brochures, flyers, newsletters, etc.), promotional materials, as well as devising mailing campaigns.
  • Manage marketing/sales budget for Home Health and Hospice.
  • Schedule and conduct meetings for prospective families and applicants. Share pertinent information regarding Athena Home Health and Hospice Services as appropriate.  Obtain any additional information needed from the family to complete an application/screen.
  • Track and evaluate referrals on a daily and monthly basis.
  • Participate in orientation and in-service training programs for professional staff.
  • Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which assist in determining policies.

  • Provides information to patients and families/caregivers and community agencies.
  • Serves as liaison between patients and families/caregivers and community agencies.
  • Maintains collaborative relationships with organization personnel to support patient care.
  • Maintains and develops contracts with public and private agencies as resources for patients and personnel.
  • Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
  • Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.
Experience & Education: 
  • Possess a master’s or doctorate degree that qualifies for licensure or certification as an MFT under State law of the State in which such individual furnishes marriage and family therapist services.
  • Is licensed or certified as an MFT by the State in which they furnish services.
  • Has performed at least 2 years or 3,000 hours of clinical supervised experience in marriage and family therapy or mental health counseling after obtaining the degree referenced above.
  • Meets other requirements as the Secretary of Health and Human Services (HHS) determines appropriate.
  • MHC must be licensed or certified as an MHC, clinical professional counselor, professional counselor, addiction counselor, or alcohol and drug counselor by the state.
  • Individuals who meet all the applicable statutory and regulatory qualifications to be an MHC --- even though they may be licensed or certified by their state under a different title to furnish mental health counseling --- may enroll as an MHC.

  • Accurately enters patient billing data, including visit charges and supply charges, into the information system.
  • Accurately processes and bills Medicare, Medicaid, private payer and patient claims in accordance with payer requirements and organization policy.
  • Assists in the preparation of monthly billing and accounts receivable reports.
  • Responsible for understanding and completing cross training on insurance verifications, reviewing information from referrals, verifying coverage and obtaining authorization.
  • Maintains accurate Medicare and Medicaid billing analysis reports.
  • Maintains complete and accurate billing and accounts receivable records.
  • Prepares Medicare, Medicaid, private payer and patient remittances for data entry.
  • Alerts appropriate management team members regarding late or missing documents required for billing.
  • Assists with the collection of receivables by monitoring accounts receivables, resubmitting bills to overdue accounts, and alerting the billing manager of seriously overdue accounts.
  • Establishes and maintains positive working relationships with patients, payers, and other customers.
  • Maintains the confidentiality of patient and organization information at all times.
  • Performs other specific projects relating to billing, data entry, and computer operations as required.
Experience & Education:
  • At least three (3) years’ experience in health care data entry, preferably in hospice care systems.
  • At least one (1) year of previous health care related billing experience, preferably in hospice care billing.
  • At least two (2) years college preferred.
