We are hiring an Occupational Therapist to join our dynamic care team at Northwood Rehab in Lowell, Massachusetts! Shift is either Sunday-Thursday or Tuesday-Saturday. As an Occupational Therapist, you provide patient/resident assessment, care planning, direct treatment and consultation to a general rehab caseload while also participating in the clinical team process. You also provide teaching and training to caregiver staff and supervise other therapists, assistants, aides and students as required. Education & Experience:
  • Bachelor of Science or Master of Science Degree in Occupational Therapy.
  • CEU’s per state licensure requirements.
  • Appropriate licensing requirements by individual states must be maintained.
Duties & Responsibilities:
  • Screens patients/residents
  • Evaluates patients/residents and develops appropriate care plans.
  • May perform additional assigned administrative duties such as:  assigning and overseeing resident care activities, medical record oversight, assuring execution of physician orders, ordering supplies, etc.
  • Under a physician’s order, provides direct resident treatments in accordance with an established plan of care.  Treatment venue includes individual, supervised and group.
  • Contributes to the team meeting and decision-making process as defined by the program.
  • Consults with and trains patients/residents and caregivers.
  • Monitors efficiencies on an ongoing basis in collaboration with supervisor.
  • Documents activities according to the company’s clinical documentation system on a timely basis.
  • Demonstrates consistent knowledge and application of Medicare and other reimbursement regulations.
  • Communicates effectively with patients, residents, team members, customers and supervisor.
  • Provides appropriate consultation to the facility.
  • Participates in clinical program development and implementation.
  • Participates in the training and orientation of other staff, students and volunteers.
  • Participates in the interviewing process for new staff.
  • Participates in census building and marketing activities.
  • Demonstrates ability for continuous new learning to remain current in the field.
  • Complies with facility Privacy Practices policies and procedures and protects residents’ individual health information.
  • May perform additional duties as requested by supervisor.
  • Participates in quality improvement activities.
  • Responsible for service and operational excellence of all assigned activities to ensure the delivery of quality services and/or outcomes required to meet or exceed the expectations of those utilizing or impacted by the department.

Duties & Responsibilities:
  • Plans, develops, organizes, implements, evaluates, and directs the nursing service department, according to state and federal guidelines and scope of patience.
  • Maintains and reviews and has input to development of all clinical policies and procedures.
  • Develops and implements a nursing service organization structure.
  • Determines the staffing needs of the nursing service department, making recommendations to the Administrator, necessary to meet the needs of the residents.
  • Oversees licensed nursing staff
  • Develops work assignments and schedules duty hours, and/or assists nursing supervisor staff in completing and performing tasks necessary to ensure quality of care.
  • Develops and maintains an on-call program for the nursing department.
  • Makes daily rounds of each nursing unit and does visual inspection on each unit.
  • Reviews satisfaction of care and services with interviews with families and residents.
  • Assists the Infection Preventionist in establishing and maintaining the Infection Control Program.
  • Assists the Director of Staff Development in ensuring that all mandatory in-service education sessions, which are required by state and federal law, are performed per schedule; documented and such documentation is retained.
  • Assists the Administrator in developing safety standards for the nursing department and monitors these through employee incidents and develops a plan of correction and audit tool for prevention. The Director of Nurses is an active member of the Safety Committee.
  • Abides by all reporting policies and measures set forth by Athena Health Care Systems.

Specific Requirements:
  • Must possess a current, unencumbered license to practice as a Registered Nurse in this state.
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such actions.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
  • Must be knowledgeable in the privacy rules and ensure all staff comply with facility privacy practices policies and procedures.
  • Must possess leadership ability and willingness to work harmoniously with professional and non-professional personnel.
  • Must possess the ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies, and procedures, etc., that are necessary for providing quality care.
  • Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to communicate effectively to appropriate personnel regarding emergency situations.
  • Must possess a comprehensive knowledge of nursing theory and practice.

  • Assesses and evaluates patient’s status by:
    1. Writing and initiating plan of care
    2. Regularly re-evaluating patient and family/caregiver needs
    3. Participating in revising the plan of care as necessary
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions.  Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures.  Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

  • Ensures that weekly and monthly statistics, patient outcome data, and billing logs are accurate, complete and reported in a timely manner.
  • Maintains sufficient staffing by responding to changes with impact staffing needs; ensure appropriate levels of professional and paraprofessional staff and maintains the clinical skill levels needed for the Program in accordance with budgetary and company guidelines.
  • Supervises program staff in daily program operations and clinical activities; monitors staff efficiencies and routinely provides feedback.  Continuously promotes interdisciplinary, clinical reasoning skills and best practices within rehabilitation.
  • Initiates the coordination of program development activities with the Administrator, Director of Nursing, Medical Director and other department directors for the purpose of introducing new and relevant services for the persons served and the community.

  • Generates and analyzes financial, operational and productivity reports as required; contributes to the development of the annual operating and capital budget.
  • Ensures that weekly and monthly statistics, patient outcome data, and billing logs are accurate, complete and reported in a timely manner.
  • Maintains sufficient staffing by responding to changes with impact staffing needs; ensure appropriate levels of professional and paraprofessional staff and maintains the clinical skill levels needed for the Program in accordance with budgetary and company guidelines.
