PURPOSE OF POSITION: Responsible as a member of the Interdisciplinary Team for the provision of nursing care for hospice patients to maximize the comfort and health of patients and families consistent with Athena Hospice of Rhode Island’s policies and procedures. ESSENTIAL FUNCTIONS: · Maintain communication with other hospice staff members and other agencies as needed to provide optimal care and use of resources for the patient/family. · Maintain regular communication with the RN Case Manager or Team Leader to review caseload. · Implement the plan of care as specified by the Interdisciplinary Team. · Provide personal care as needed. · Perform nursing procedures as ordered. · Determine the patient's needs and report findings to the RN Case Manager. · Recommend changes to the plan of care as appropriate. · Complete accurate charting of findings, treatments, and patient responses. · Organize care to provide timely and efficient care to all patients. · Supervise nursing aids. · Provide emotional support to patients and their family and friends. · Follow strict infection control guidelines. · .Participate in agency and community programs as requested to promote professional growth and understanding of hospice care. · Demonstrate familiarity with policies of the agency and rules and regulations of State and Federal bodies which assist in determining policies. EDUCATIONAL REQUIREMENTS Possession of current license to practice as a registered professional nurse in the state of Rhode Island. Preferably a B.S. degree in nursing. Minimum two years of varied work experience as a professional nurse and preferably one of the two years in an acute or hospice setting. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Demonstrated skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. EXPERIENCE A minimum, one (1) year experience in a hospital, long-term care facility or other related health care facility is preferable. SPECIFIC REQUIREMENTS:
  • Coordinate all admissions to facility in accordance with budgeted census projections.
  • Is readily available and represents the facility to potential residents, families, hospitals, and other referral sources in a positive and professional manner.
  • Acts as a liaison between facility and all community groups and agencies.
  • Works with Administrative staff to plan, develop and implement facility marketing plan.
  • Utilize appropriate sales/marketing techniques (i.e., public speaking, community workshops, cold calling, facility events, media coverage, etc.).
  • Maintain budgeted census mix.
  • Assure detailed completion of all admission, discharge, and transfer records manually as well as computer generated. Has knowledge of admission software and able to utilize efficiently including electronic referral systems.
  • Serves as a community contact between facility and community-based organizations, clubs and civic events including but not limited to Rotary, Chamber of Commerce and or other community associations.
  • Research and identify all community groups and organizations to determine specifically which groups to target to reach the facility’s goals and marketing strategies.
  • Invite key groups to facility for joint activities, functions and special events. Allow groups and clubs to hold regular meetings at facility.
  • Demonstrates the ability to facilitate, prioritize and coordinate census improvement strategies within the community and maintains daily community contacts.
  • Provide ongoing assistance to potential residents and their families in negotiating a successful placement.
  • Able to train and orient staff to admission policies and procedures.
  • Must be trained as the back up to the Clinical Liaison and be credentialed at any and all local hospitals.
  • Must be able to take on call responsibilities including holiday, night and weekend coverage.
EXPERIENCE: Minimum of two (2) years’ experience in an Admission Director position in a skilled nursing facility or assisted living facility. Must have demonstrated capabilities for management, administration, and supervision. Demonstrated leadership qualities. Must have thorough knowledge of local, state, and federal rules and regulations as they pertain to admissions. EDUCATION: Minimum of a Licensed Practical Nurse or Registered Nurse preferred, or a related field approved by Senior Management.   • Must possess accurate and comprehensive assessment skills to ensure standards of nursing practice.

  • Must be able to cook a variety of foods in large quantities.
  • Must be able to read, write, speak and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must be knowledgeable of dining service procedures as well as related laws, regulations and guidelines pertaining to hospital and nursing home operations.
  • Must possess leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel.
  • Must maintain the care and use of supplies, equipment, the appearance of work areas, and perform regular inspections of food service areas for sanitation, order, safety and proper performance of assigned duties.
  • Must have patience, tact, cheerful disposition and enthusiasm as well as be willing to handle residents based on whatever maturity level they are currently functioning.
EXPERIENCE:
  • One (1) year dietary experience in a hospital, skilled nursing care facility or other related medical facility preferred, but not necessary.
