As the Bereavement Coordinator, you plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Athena Hospice for up to one (1) year following the death of the individual hospice patient.
Experience & Education:
- Bachelor’s degree or equivalent theological degree from college or university. Experience with families/caregivers and patients through death/dying issues – OR – Master’s degree in Social Work with qualifications of experienced clinical grief counseling.
- Knowledge and commitment to hospice philosophy.
- Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.
- If making home visits, must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
- Ability to travel.
- Coordinates the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
- Plans for Bereavement Program development, expansion, and refinement through annual evaluation of services.
- Provides bereavement support to hospice personnel coping with work related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.
- Provides bereavement information and referral services to callers from the community. Expands and maintains community resource list as needed.
- Assists as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups).
- Functions independently but seeks and accepts guidance from other members of the interdisciplinary group, or from members of the community.
- Participates in hospice activities such as Inservice education, hospice personnel meetings, and relevant committees.
- Advances professional knowledge and skills.
- Actively participates in quality assessment performance improvement teams and activities.
- Accepts and performs other related duties and responsibilities as required.

In this role, you are responsible for the provision of spiritual care services to patients and families/caregivers, either directly or through coordination of care with other spiritual counselors.
Education & Experience:
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree).
- Experience working with death and dying, individuals/family/caregiver.
- Ability to work as member of interdisciplinary group.

- Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
- Provides consultation, education, and support to the interdisciplinary group on spiritual care.
- Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
- Provides for funeral or memorial services for patients as requested.
- Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
- Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers.
- Actively participates in quality assessment performance improvement teams and activities.

As a member of our hospice team, you carry out duties assigned by the case manager to increase patient comfort, to maintain good personal hygiene, and a safe environment for the patients.
Education and Experience:
- Pass the Home Health Aide competency exam.
- Adheres to the written home health aide plan of care as prepared by the RN Case Manager.
- Promptly reports changes in the patient’s condition to the RN Case Manager.
- Assists the patient with all aspects of personal care, ambulation and positioning as required.
- Prepares meals and assists with feeding as required.
- Performs household services which are necessary for the health and safety of the patient and family.
- Provides assistance with bedpans and urinals as needed.
- Provides guidance to the caregiver regarding personal care of the patient.
- Attends and participates in weekly team meetings, AM report, inservices, and agency meetings and other related activities when appropriate.
