As the MDS Coordinator, you plan and organize the MDS process in accordance with federal and state standards, guidelines, and regulation that govern our facility. Experience & Education:
  • Must possess, as a minimum, a Nursing Degree from an accredited school of nursing, college or university.
  • Must possess a current, unencumbered license to practice as an RN/LPN in this state.
Duties & Responsibilities:
  • Coordination of MDS process
  • Oversees ADL training for facility and staff.
  • Issues and delivers denial notices timely and appropriately.
  • Complete and transmit all CMS approved item sets (MDS)
  • Must be knowledgeable of and follow current CMS regulatory guidelines as described in RAI Manual.
  • Complies with facility privacy policies and procedures and protects residents’ individual health information.
  • Maintains Medicare meeting minutes per Medicare program agreement.
  • Issues and delivers Medicare denial letters per CMS regulations.
  • Assures appropriate management of residents’ Medicare/Insurance benefits.
  • Maintains adequate systems to ensure appropriate documents are sufficient to support billed services.
  • Other duties assigned by manager.
Specific Requirements:
  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such an action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public.
  • Must be able to coordinate MDS systems, resident assessment, and care plans for each resident timely.
  • Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care.
  • Must possess leadership and supervisory ability and the willingness to work harmoniously with professional and non-professional personnel.
  • Must have patience, tact, a cheerful disposition, and enthusiasm, as well as the willingness to handle difficult residents.
  • Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
  • Must be able to communicate effectively to appropriate personnel regarding emergency situations.
  • Must possess accurate and comprehensive assessment skills to ensure standards of nursing practice.

  • Ability to demonstrate technical skills which support/enhance sales skills and results through sales delivery, collateral use, product knowledge, knowledge of competitors, identifying challenges and creating/initiating solutions in a positive and effective way.
  • Responsible for external sales calls/contact with assigned referral accounts based on account priority listing.
Specific Requirements:

  • Accurately enters patient billing data, including visit charges and supply charges, into the information system.

  • Alerts appropriate management team members regarding late or missing documents required for billing.

  • Establishes and maintains positive working relationships with patients, payers, and other customers.
  • Maintains the confidentiality of patient and organization information at all times.

As a Resident Care Coordinator Assistant, you will assist the Resident Care Coordinator in the day-to-day activities of the Resident Assessment Process in accordance with current federal, state, and local standards, guidelines and regulations that govern our facility, and as may be directed by the Resident Care Coordinator Director/MDS Director. Experience & Education:
  • Previous experience with the MDS process is preferred.
  • Active Certified Nursing Assistant (CNA) license and experience is required
  • Knowledge of medical terminology, filing, typing, computer, and organizational skills.
  • A minimum of a high school diploma and demonstrated capabilities of clerical skills with an emphasis on organization and ability to prioritize.
Duties & Responsibilities:
  • Assists with monitoring of ADL Flow Sheets with observation of ADLs to ensure accuracy of coding.
  • Provides education to CNAs on ADL coding.
  • Maintains tracking form to evaluate progress of CNAs who have been identified as needing on-going education and provides the information to the RCC/MDS Director so that he/she can follow up.
  • Enters demographic information into Minimum Data Sheet (MDS) software system.
  • Completes discharge and re-entry MDS forms.
  • Opens MDS software system.
  • Schedules resident care plan meetings and sending out invitations to family members.
  • Coordinates teleconference meetings with family members, if requested.
  • Schedules OBRA MDS for all LTC residents.
  • Follows up with interdisciplinary team members on non-completed sections of MDS.
  • Retrieves/Prints and faxes medical records to managed care companies for utilization review purposes.
  • Transmits MDSs to the State database.
  • Prints and files MDS, CAAs and care plans.
  • Retrieves QM reports and emails to facility and Athena staff.
  • Reproduces care plans that have been edited by interdisciplinary team members.
  • Tracks and schedules all Medicare certifications and recertification.
  • Mails and tracks all Medicare denial letters.
  • Retrieves/prints and faxes medical records to QIO for any requested appeals.
  • Assists with compiling medical records and preparing for submission with subsequent level appeals to QIC, ALJ, etc.

  • Must posses the ability to deal tactfully with personnel, residents, family members, visitors, and the general public.
