As a Marriage and Family Therapist/Mental Health Counselor, you:
- Assesses the psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient’s identified residence and assistance when this is not safe and another plan is required.
- Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels’ patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
- Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
- Maintains clinical records on all patients referred to social work.
- Educates patients and families on, and assists in, preparation of advanced directives.
- Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
- Provides information to patients and families/caregivers and community agencies.
- Serves as liaison between patients and families/caregivers and community agencies.
- Maintains collaborative relationships with organization personnel to support patient care.
- Maintains and develops contracts with public and private agencies as resources for patients and personnel.
- Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
- Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
- Actively participates in quality assessment performance improvement teams and activities.
- Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.
- Possess a master’s or doctorate degree that qualifies for licensure or certification as an MFT under State law of the State in which such individual furnishes marriage and family therapist services.
- Is licensed or certified as an MFT by the State in which they furnish services.
- Has performed at least 2 years or 3,000 hours of clinical supervised experience in marriage and family therapy or mental health counseling after obtaining the degree referenced above.
- Meets other requirements as the Secretary of Health and Human Services (HHS) determines appropriate.
- MHC must be licensed or certified as an MHC, clinical professional counselor, professional counselor, addiction counselor, or alcohol and drug counselor by the state.
- Individuals who meet all the applicable statutory and regulatory qualifications to be an MHC --- even though they may be licensed or certified by their state under a different title to furnish mental health counseling --- may enroll as an MHC.

As a Client Service Coordinator, you will play a crucial role in providing exceptional support to our clients and ensuring the smooth operation of various client-related processes. Your primary responsibilities will revolve around data entry, insurance eligibility verification, scheduling, maintaining documentation, intake procedures, and delivering outstanding customer service through phone interactions. Essential Job Functions:
- Manages all daily patient referral and intake operations and ensures insurance compliance with all state and regulatory requirements.

- Coordinates with Registered Nurse to verify clients' insurance coverage and benefits to ensure proper billing and coverage for services provided. Collaborate with insurance providers to resolve any discrepancies and assist clients with insurance-related inquiries.
- Scheduling: Manage client appointments and scheduling requests, ensuring that appointments are scheduled efficiently and in a manner that meets both client and business needs. Handle rescheduling and cancellation requests promptly and professionally.
- Maintaining Documentation: Create and maintain comprehensive and up-to-date client records, files, and documentation. This may include intake forms, treatment plans, provider orders, progress notes, and any other required paperwork.
- Intake Procedures: Guiding the referring source through the intake process, collecting necessary information, explaining company policies, and answering any initial questions or concerns they may have faxing physician consents timely.
- Customer Service with Phone Calls: Provide exceptional customer service to clients via phone calls. Address inquiries, provide information, and assist with problem resolution in a friendly and professional manner. Strive to create a positive and empathetic customer experience during every interaction.
- Manages all Durable Medical Equipment requests for clients, including data entry, ordering, and providing notifications of patient discharges.
- Manages order processing for all hospice/ home health physician orders.
- Collaborates with other departments and team members to ensure a seamless experience.
- Assist with Administrative tasks as needed, including filing, scanning and mailing documents.
- Other duties as assigned by the Director.
- Minimum of two (2) years’ experience preferably in hospice or health care, or graduation from a one (1) or two (2) year business college preferred.

- Accurately enters patient billing data, including visit charges and supply charges, into the information system.
- Accurately processes and bills Medicare, Medicaid, private payer and patient claims in accordance with payer requirements and organization policy.
- Assists in the preparation of monthly billing and accounts receivable reports.
- Responsible for understanding and completing cross training on insurance verifications, reviewing information from referrals, verifying coverage and obtaining authorization.

- At least three (3) years’ experience in health care data entry, preferably in hospice care systems.
- At least one (1) year of previous health care related billing experience, preferably in hospice care billing.
- At least two (2) years college preferred.
