We are hiring a Memory Care Program Coordinator to join our dynamic recreation team at Abbott Terrace in Waterbury! This is a full-time position with weekly pay starting at $30/hour. We strive to create a workplace where your skills and talents are nurtured to allow you to grow within the com As the Memory Care Program Coordinator, you enhance the overall operation of the dementia unit by developing/implementing the dementia specific programming to ensure that an ongoing program of activities designed to meet, in accordance with the comprehensive assessment, the interests and the physical, mental and psychosocial well-being of each resident is maintained. Education & Experience:
  • Must possess, as a minimum, a certificate in Therapeutic recreation from an accredited college or Associate Degree with major emphasis in Therapeutic Recreation.
  • A Bachelors Degree in a related field and one year of full time employment in Therapeutic Recreation in a health care facility; or a Bachelors Degree in a related field and 6 credit hours of Therapeutic Recreation coursework; or an Associates Degree in a related field and two years full time employment in Therapeutic Recreation in a health care setting; or an Associates Degree in a related field and 9 credit hours of Therapeutic Recreation coursework as required by the State employed in. A related field shall include, but is not limited to Sociology, Psychology, Art, Music and Education.
  •  Certified Dementia Practitioner required.
  • As a minimum, one (1) year experience in a supervisory capacity in a hospital, skilled nursing care facility or other related medical facility activities department is preferred.
  • Experience working with those diagnosed with Dementia or Alzheimer’s required.
Duties & Responsibilities:
  • Responsible for developing a purposeful and engaging memory care program for residents in the dementia unit.
  • Supervise program aides and therapeutic recreation directors that assist with programs in the dementia unit.
  • Complete care plans and documentation in a timely fashion.
  • Assist Director of Staff Development in facilitation of monthly dementia in-services and Hand in Hand for all new employees, with materials provided by Athena.
  • Assist in plans and provisions of patient care by visiting residents regularly to evaluate needs.
  • Document and report to the nursing department any unusual or significant findings.
  • Report all reportable events immediately in accordance with our policy.
  • Encourage resident to take part in activities of daily living that will stimulate his/her potential, physically, mentally, socially and spiritually.
  • Spend duty time with residents rather than with other facility staff
  • Assist with and follow through on the establishment of resident care goals within the framework of the entire therapeutic team.
Specific Requirements:
  • Must possess sensitivity to those diagnosed with Dementia and their families.
  • Must have an understanding of appropriate activities for those with dementia.
  • Must be able to read, speak and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and general public.
  • Must be knowledgeable in the privacy rule and ensure all staff comply with facility privacy practices and policies and procedures.
  • Must possess leadership ability and willingness to work harmoniously with and supervise professional and non-professional personnel.
  • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to work with residents at varying functioning levels.
  • Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing program.
  • Must be able to relate information concerning a resident’s condition.
  • Must participate in continuing education and inservice training.

  • Determines the staffing needs of the nursing service department, making recommendations to the Administrator, necessary to meet the needs of the residents.
  • Oversees licensed nursing staff

  • Develops and maintains an on-call program for the nursing department.

  • Assists the Infection Preventionist in establishing and maintaining the Infection Control Program.
  • Assists the Director of Staff Development in ensuring that all mandatory in-service education sessions, which are required by state and federal law, are performed per schedule; documented and such documentation is retained.
  • Assists the Administrator in developing safety standards for the nursing department and monitors these through employee incidents and develops a plan of correction and audit tool for prevention. The Director of Nurses is an active member of the Safety Committee.
  • Abides by all reporting policies and measures set forth by Athena Health Care Systems.

  • Follows Pharmacy policies and procedures. Maintains an accurate and safe control of all drugs for which he/she is responsible.
  • Ensures that CNA’s follow established Infection Control Procedures to prevent the spreading of infection. Demonstrates correct safety techniques.
  • Completes Accidents and Incidents reports (Reportable events), ensuring that all appropriate notifications, documentation, investigation, reporting, and interventions are complete.
  • Initiates any and all emergency/disaster procedures as necessary. Be familiar with and ensure that staff adheres to safety, health, fire, disaster, emergency, sanitation codes and MSDS.
